Our Caregivers consist of State certified CNA’s and HHA’s, trained and highly competent in assisting our clients with their Activities of Daily Living (ADL’S). Personal Care Services may include:

+ Transportation Assistance

When a client is recovering from a minor illness or needs minimal assistance to remain secure at home, there is still a need to travel around town or running errands.  Whether going to and from appointments, visiting friends and relatives, or whatever the event may be, we will assist you in accommodating your transportation needs.  We can provide the staff who can accompany you in getting to your destination and going back home after the day is done.  We will accompany you to all your outings and also make sure you return home and settled in safely.

There will be an additional fee assessed to the client for this service.  Clients who prefer not to pay for these additional charges may choose to take alternate transportation means, such as using their own vehicle (with the caregiver driving) or using public and/or private transportation, such as the Handivan.

Whether you are traveling out of state or around the globe for an appointment or for whatever purpose, having someone who is compassionate, that is dependable, and one you can trust by your side to assist you, can make life so much easier.

Having our professional Aide to accompany you in these trips can make the experience so much more tolerable and comfortable.  In the event that you need professional care, there’s always someone you can turn to – even if you’re away from home.

We bring the professional care service while traveling to your destination and when arriving there.  You can have the peace of mind that there’s a professional aide ready to assist with your health care needs when needed.

+ Respite Care for the Family Caregiver

We understand that caring for a loved one can be very stressful.  It can be physically and emotionally straining even to the most dedicated Family Caregiver.  That is why knowing about Respite Care is so important.

You can only give the best quality service to your loved one when you are at your best.  Occasionally, you need to take some time-off from the routine of care giving.

Having respite care not only provides relief, but when used regularly, can actually improve the quality of the family caregiver’s life.  They can do things that were limited by their responsibilities for their loved one.

+ Live-In/24 Hour Coverage

Live-In/24 Hour CoverageThere may be instances when around-the-clock assistance is needed for a loved one.  Whether the need is to support the continuity of care in the home after being discharged from the hospital or an institutional facility during convalescent period or you are someone who is chronically ill and find yourself challenged with your daily activities.

InfiniteCare-Plus specializes in 24 hours a day 7 days a week, live-in care for our elderly and others in need for this type of service. Our Caregivers come to the home or senior facility to provide any or all of the following services:

  • Meal Preparation
  • Light Housekeeping
  • Laundry
  • Linen Changes & Bedmaking
  • Grocery Shopping & Running Errands
  • Companionship
  • Ambulation & Range of Motion Exercise
  • Assistance with Bathing, Dressing, Grooming, etc
  • Transportation assistance to Doctor’s Appointment, Supermarkets, Pharmacy, etc
  • Status Reporting to family, Case Managers, Physicians

Whatever your need for a live-in/24 hour care service, we are ready, willing and able to accommodate you.  Our live-in/24 hour care service is charged at a flat daily rate.

This is how live-in care works: A live-in Caregiver stays in the home 24 hours a day, and is available to assist morning, noon and night. If we provide live-in care for 7 days a week, we assign a Primary Caregiver and a Back-up Caregiver to the job. The Primary Caregiver usually provides care for 4 to 5 days a week and the Back-up Caregiver takes over for the remainder of the week.

All live-in Caregivers must maintain a separate, permanent residence. They are permitted to reside on the client’s premises only during their assigned days and nights, and they must leave the client’s premises with their belongings on their days off. They are strictly forbidden from using the client’s property, such as phone, television or car, for their own, personal use. Live-in Caregivers must also respect the company policy on visitors: None are allowed while providing live-in care.

We offer a wide range of skilled nursing services ranging from Licensed Practical Nurses (LPN) to Registered Nurses (RN).  Some of the tasks are as follow:

+ Care Management

Care ManagementCare Management is a collaborative approach to providing and coordinating you or your loved one’s health care services.  Our Care Manager will work with you, your family, physicians and other health care providers to ensure you or your loved one receives all the necessary services to promote your health and oversees any illness or health conditions while you or your loved one continue to live at home.

Our Care Manager serves as your advocate and liaison between yourself and all providers of your health care services.  In today’s health care environment, it can be overwhelming to navigate the system.  You may not know all the questions to ask the physician or you may not be familiar with the vast array of services available to meet your needs.  Medical language is often unfamiliar and physicians cannot spend enough time with you to help you understand all the complexities of your illness.

Our Care Manager bridges that gap to assure that you have all the information you need to make well-informed health care decisions.  Additionally, our Care Manager provides linkages to other resources and services to assure you or your family member has all their needs cared for.

Our Care Manager helps you understand and cope with the medical, insurance, and emotional issues confronting you.  A comprehensive assessment is performed and recommendations to enhance your health care and living situation are provided.  A clear plan for coordinating your health care and maintaining or improving your health is provided.

Our Care Manager’s Functions:

  • Prevents unnecessary services or treatment.
  • Evaluates an individual’s safety and ability to live independently at home.
  • Obtains appropriate medical care.
  • Secures necessary medical supplies and equipment.
  • Obtains home care nursing services.
  • Obtains assistance with homemaking and personal care needs such as bathing, dressing, and grooming.
  • Obtains assistance with running of errands.
  • Coordinates health care service delivery and necessary medical follow-up.
  • Arranges for transportation to and from doctor appointments.

THE CONTINUUM OF CARE in Conjunction with our Care Management

To meet our client’s constantly changing needs, we strive to make our care services as flexible and as versatile as possible. Through our Care Management process, our Caregivers are fully trained and dutifully required to observe, monitor, and document all aspects regarding their client’s health condition.

Our Caregiver will report any concerns to the client’s family members, our Care Manager, Primary Care Physicians, and/or guardians. Daily documentation is always available for review in the Care Plan, and can also be transmitted electronically as needed.

If our Care Manager determines that the client’s care needs have surpassed our service capabilities, we will immediately notify the family of the need for a change in coverage. Through this proactive approach, you can rest assured that you or your loved one will receive the appropriate, qualified level of care.

In addition to the convenience, time-saving and security they provide, our Care Manager can save your family money because needs assessments align an individual’s present condition with only those services that are necessary at that time.
